Download the required forms below. You can enter the information online, and print the completed form. Turn the required forms into your 4-H Club Leader or your Ag Science Teacher before the deadline.

Deadline to submit: October 17, 2008 at 5:00 pm

• Broiler Order Form
  Required for all participants who will be showing Broilers at the Nueces County Junior Livestock Show

Deadline to Submit: October 31, 2008 at 5:00 pm

• Livestock Entry Form
  Required for all participants in the Livestock Divisions of the Nueces County Junior Livestock Show
• Homemaking Entry Form
  Required for all participants in the Homemaking Division of the Nueces County Junior Livestock Show
• Horse Show Validation Form
  Required for all participants in the Nueces County Junior Livestock Show Horse Show (1 per horse entered)
• Queen's Contest Entry Form
  Required for all Nueces County Junior Livestock Show Queen Contestants

• Agreement To Arbitrate
  Required statement concerning alternative dispute resolution for exhibitors in the Nueces County Junior Livestock Show
• Residency Request
  Required of projects raised and/or housed outside of Nueces County
• IRS W-9 Form
  Required for all entries. No auction or other payments will be made to an exhibitor unless a W-9 is on file.
  Additional information about filling out W-9 forms can be found at the Internal Revenue Service website.
• Minor's Release Form
  Required statement concerning release of liability
• Drug Certification Form
  Required for all market livestock show exhibitor
